Mayor's Celebratory Certificate Request Form

Please allow 2 – 3 weeks for completion of certificate.

Mayor's Celebratory Certificates are custom certificates issued and signed by the Mayor to celebrate milestone birthdays, wedding anniversaries and non-profit organization anniversaries. Certificates are issued for 70th birthdays and every 5 years after that (i.e. 70, 75, 80, 85, 90, 95, 100). Anniversary Certificates are issued for 40th wedding anniversaries and every 5 years after that (i.e. 40, 45, 50, 55, 60, 65, 60 etc). Non-profit anniversary certificates issued for 25th anniversaries and every 5 years after that (i.e. 25, 30, 35, 40, 45, 50, 55, 60 etc).
